In addition to the online registration process, the following documents and forms must be turned in either during our onsite registrations or during equipment handout. It is especially important for Cheer Participants to attend one of the onsite registration events to be properly sized for uniforms, shoes, etc.
Documents and Forms due as follows:
- Football or Cheer Registration Form
- Parent and Cheerleader Rules and Regulations (Cheer only)
- No Refund Form
- Participant Contract Form
- Husky Family Agreement
- Little Scholars Form
- Volunteer Form
- Birth Certificate (show original with certified seal plus 1 copy)
- 2021 Pop Warner Physical Form dated after Jan. 1, 2021
- 2020 - 2021 Final Report Card (show original plus 3 copies)
Failure to provide any of the forms will result in your child not being able to participate until the missing documents are turned in. This is a mandatory National Pop Warner requirement.
